Job Description:
A company is looking for a Bilingual (Spanish) Transitions of
Care Nurse Practitioner.
Key Responsibilities
Perform initial transitions of care visits, including comprehensive
assessments and medication reconciliation
Conduct virtual urgent care visits to diagnose and manage acute
symptoms
Participate in interdisciplinary rounds to discuss high-risk
patients and enhance team culture
Required Qualifications
At least 3 years of experience in a hospital, post-hospital, or
primary care setting
Bilingual in English and Spanish
Must be willing to work five 8-hour shifts (Monday-Friday)
Active RN and APRN licenses in good standing
Active BLS certification at the time of hire
