A company is looking for a Care Navigator - Population
Health.
Key Responsibilities
Implements care plans tailored to patients' needs based on clinical
assessments
Coordinates follow-up care for patients not meeting clinical goals
and manages documentation in the electronic health record
Collaborates with multidisciplinary teams to provide education and
navigation services for patients and families
Required Qualifications
Bachelor's Degree in Nursing or equivalent experience
5 years of experience as a registered nurse (RN)
2 years of experience in case management, disease management, or
related settings
2 years of electronic medical record (EMR) experience
Licensed Registered Nurse in Pennsylvania upon hire
