Registered Nurse (Wellness Center)

Job Description:

The Registered Nurse is the key to an integrated and seamless patient experience in our wellness centers. In this position, nurses are expected to demonstrate clinical leadership, participate in patient care activities, proactively assist with ongoing health promotion, and support the needs of the primary care physician. Duties also include phlebotomy, specimen handling and processing, point of care testing, and biometric screenings. This position will have oversight also for medical supply procurement, and pharmaceutical procurement in addition to having product knowledge to facilitate sales.

Essential Functions:

  • Delivers direct patient care and physician support
  • Provides evidence based nursing and patient education at the highest community standard
  • Coordinates and connects the patient with specialists and facilities
  • Development of disease and condition specific clinical care programs
  • Provide Phlebotomy Services
  • Support the Wellness Center---s patient service excellence standards by leading, motivating, providing consistent feedback and acting as a positive role model. Additional Responsibilities:
    • Performs all other duties as assigned. Age-Related Competencies:
      • Human development knowledge/skills: Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served including knowledge of the principles of growth and development and psychosocial characteristics and interventions for age specific requirement of the assigned unit.
      • Age-specific patient needs that employee is required to understand and meet: Demonstrates the ability to assess and interpret age appropriate data about the patient---s status in order to identify age-specific needs and provide the care needed.
      • Additional requirements: Demonstrates age appropriate communication skills for the patient population served. Demonstrates knowledge of age-specific community resources. Information Management: Treats all information and data within the scope of the position with complete confidentiality and security. Maintains all practices to protect PHI and follow HIPAA guidelines. Adheres to clients confidentiality and security requirements in addition to employers.

        Risk Management: Cooperates fully in all risk management activities and investigations.

        Safety Requirements: Is knowledgeable of, observes and complies with ambulatory Codes of Safe Practice, safety policies and emergency procedures of employer and client.

        Minimum Position Qualifications:
        Education: Requires a Bachelor of Science in Nursing (BSN)

        Experience: Five (5) year---s previous experience as a Registered Nurse required.

        License / Certification:
        • Current California RN license
        • Current BLS certificate Computer Skills: Proficient in a Windows and or Mac operating environment including Word, Excel, PowerPoint, and email.

            • Previous experience as a Health and Wellness Coach highly desirable
            • Ambulatory or worksite/employee health experience preferred License / Certification:
              • Certified fitness and/or yoga instructor a plus
              • Certified Health and Wellness Coach preferred. Computer Skills: Previous EMR experience highly desirable

                Other: Multi-lingual highly desirable

                Environmental Conditions:
                May include exposure to potentially hazardous material, infectious agents, and contact with patients, public and staff.

                Physical Requirements:
                Standing for long periods of time, bending stopping, kneeling, pushing/pulling, and lifting/carrying grasping. Ability to travel between SJHH locations.
