Job Description:
A company is looking for a Discharge Care Management Nurse RN to
work remotely in the Central or Eastern Time Zone.
Key Responsibilities
Evaluate member discharge needs and develop readmission prevention
plans
Collaborate with providers and members to coordinate post-discharge
care
Document discharge activities and participate in team meetings and
discussions
Required Qualifications
Bachelor of Science in Nursing
Current, unrestricted Compact RN license in the State of
residence
5+ years of recent experience in an inpatient acute setting
2+ years of experience in discharge planning
Experience with multiple health insurance products and compliance
requirements
