Job Description:
A company is looking for a Clinical Pharmacist - Remote.
Key Responsibilities:
Conduct clinical chart reviews and interpret clinical information
to optimize medication regimens
Utilize reporting tools to identify non-adherent members and
present findings to clinical teams
Provide education on disease state management and lead meetings to
improve medication adherence performance
Required Qualifications:
Doctor of Pharmacy degree or equivalent experience
Active, unrestricted pharmacy license
5+ years of Pharmacy Practice experience, including 2+ years in
Clinical Practice
Experience in a clinical setting such as Managed Care, Clinic, or
Hospital
Comfortable making clinical recommendations based on clinical
guidelines
