Lead Health Coach





Location: Irvine, CA


Work Schedule: Part Time - 40 Biweekly Hours


Shift: 8-hour, Days



This position provides health coaching, with an emphasis on nutrition and lifestyle, in the community. The Lead Health Coach functions independently at the wellness center under direction of the medical director in providing group, individual and phone coaching in various settings at the wellness center. The Health Coach will need to work in collaboration and support of onsite clinic providers to develop nutritional meal plans, program and personal nutrition recommendations for clients. In addition, this position is responsible for researching, developing, implementing, tracking and evaluating health improvement programs and outcomes in alignment with St. Joseph Heritage Healthcare's wellness philosophy of care.



Required qualifications:

• 
  
• Bachelor's Degree in Nutrition, Science, Education or related Health field.
  
  
• 1 year working experience as a nutritionist, dietitian or health coach in an outpatient healthcare setting.
  
  
• Experience with providing MNT and individual counseling.
  
  
• Experience with teaching and modeling self-management skills, concepts, resources and tools related to health, lifestyle change and behavior modification to class participants.
  
  
• Experience with working with patients with chronic diseases and other primary prevention programs.
  
  
• An active CPR Certificate.
  
  
  
  
  
  Preferred qualifications:
  
  
  
  
  
  • 
    
  • Master's Degree in Nutrition.
    
    
  • 2 years working experience as a Nutritionist, Dietitian or Health Coach in an outpatient healthcare setting.
    
    
  • AEMR, IDX, Allscripts, PowerPoint and Excel experience.
    
    
  • Certified Clinical Nutritionist or Registered Dietitian certified by the Commission on Dietetic Registration as defined by the California Business and professional code and be current with CU/CDR portfolio.
    
    
  • Functional Medicine Certified Health Coach.
